I acknowledge that I, on behalf of myself, my next-of-kin, my heirs, executors, administrators and assigns, hereby release and discharge the World Transplant Games Federation (WTGF), and any other representatives involved in the creation of Refit for Life for any injury, loss, or damage to my person or property and all expenses and costs, however caused, arising out of, or in connection with my participation in the Refit for Life Rehabilitation Program. I agree that by participating in this program I am personally assuming responsibility for any injury to myself or my property and I acknowledge that I have been medically cleared to participate in this program. I furthermore acknowledge that by participating in this program, the WTGF may collect personal data pertaining to myself, my transplant and my progress on the Refit for life program. The personal data, which may be kept on paper or on a computer or other media, is subject to legal safeguards specified in numerous laws and regulations of its member countries.
